Appalachian is not a language similar to any RL Appalachian dialect of English, but rather a fictional language similar to old English spoken in the Southmountain Confederation, a nation that geographically and politically replicates south central and southwestern Pennsylvania, along with Western Maryland and northeastern West Virginia.
